BREAST AUGMENTATION BY IMPLANT

The breast has always aroused great interest. This is hardly surprising, given that the breast alone symbolizes femininity, maternity and sexuality.

The unloved breast is difficult for a woman to live with, as it hinders her development and identity as a woman.

Whether your breasts are too small or too large, sagging or malformed, it's possible to have a specialized consultation for advice and a solution.

 

What are the goals of breast augmentation with implants? 

Breast augmentation is cosmetic surgery to increase breast volume.

In addition to improving your self-confidence and body image, breast augmentation offers many advantages:

give you fuller breasts and increase their projection.

harmonize the size and shape of asymmetrical breasts.

restore breasts to their size before breast-feeding or major weight loss.

 

How many consultations are required?

 

Two consultations are required before breast augmentation surgery. 

The first consultation; we discuss your expectations and desires regarding your breast augmentation, so that we can offer you the solution best suited to your needs. 

On the first try, you will find your ideal breast size.  with specially designed trial prostheses to determine your future breast size. 

During the second consultation, we will validate the size of the prostheses and the location of the future scar, as well as the type of conventional or outpatient hospitalization.

 

What types of breast implant:

Breast implants are usually measured in volume, not cup size. The final volume is determined by combining the base width and height of your breasts with the profile you desire.  It's important to choose the right width and height for your breasts to achieve a natural look.  The profile (or projection) is the distance at which your implants will contour from your thorax. Choosing the right volume, width, height and projection is essential to achieving the result you're looking for. A larger implant does not necessarily mean that your breasts will appear larger - it all depends on your morphology. The size and shape of your breast implants will be as unique as your figure.

Round or anatomical?

ROUND IMPLANTS

Over time, the upper poles of the breasts can lose their curvature due to gravity and appear flatter. Round implants fill out the existing breast by adding volume to the upper pole.

We can achieve very natural results with round implants, the texture of which can be smooth as there is no risk of rotation.

ERGONOMICALLY SHAPED IMPLANTS

Round implants that assume an anatomical shape in the vertical position.

ANATOMICAL OR TEARDROP-SHAPED IMPLANTS 

Anatomical or "teardrop" implants fill out existing breast tissue, subtly augmenting the upper pole and, more markedly, the lower pole to match the natural curves of the figure. They can also help to shape the breast when little tissue is present. Their major drawback is that they must be coated with a macro-texture to avoid the risk of rotation. This macro-texture is responsible for a one-in-100,000 risk of lymphoma.

Silicone implants:

Fixed-surface, fixed-volume breast forms pre-filled with silicone gel. To avoid deformation caused by displacement of the prosthesis, smooth prostheses are always round. There is no risk of silicone gel migration in the event of prosthesis rupture.

> Reduces the appearance of wrinkles

> Eliminates the risk of deflation 

>Consistency close to that of a normal breast

Polyurethane prostheses:

The implant must retain its shape, elasticity and flexibility.

To avoid this fibrosis, 2 laboratories have developed implants coated with a micropolyurethane foam. Tissue can actively penetrate the micro-ventilated foam, interrupting the linear structure of the collagen fibers. It is not the implant itself but the foam structures that are encysted, so that contracture of the soft tissue fibers does not lead to hardening and deformation of the implant.

Polyurethane breast implants show a capsular contracture rate of 0 to 3%, compared with rates of 9 to 20% for other surfaces.

Saline prostheses:

Saline-inflated prostheses are filled with sterile salt water. In the event of rupture of a saline-filled prosthesis, the saline solution will be absorbed and naturally expelled by the body, resorbing safely back into the body.

> Natural absorption by the body in the event of envelope rupture

>  Fewer retractable "shells" (hardening).

Hydrogel prostheses:

> Perfectly biocompatible and biodegradable filler!

> Envelope with almost 2 times the elongation capacity of silicone gel implants.

> For the patient, there's no need for an MRI to detect a rupture: the breast will deflate slightly, almost painlessly.

> Hydrogels are totally transparent to X-rays, making mammography interpretation as easy as on a non-implanted patient.

 

HOW DOES THE PROCEDURE WORK?

The surgeon makes pre-operative drawings on the day of the operation, and this is also a time for sharing with the patient the surgical schedule.

We begin breast augmentation by making an incision along the crease beneath the breast, or around the lower areola of each breast. 

After the incision, the surgeon carefully creates the space where the implant will be inserted. Most of the time, the surgeon will create a DUAL PLAN space, i.e. half under the muscle and half under the gland. Once the surgeon is certain that the size and shape of your new breasts meet the desired aesthetic results, he or she closes the incision with absorbable sutures.

Dr. David does not use a drain, so there will be no drain to remove after the procedure.

IMPLANT POSITION?

Dr. David uses the Dual Plan technique in the vast majority of cases. In the Dual Plan technique, the breast implant is placed half under the muscle on the upper, inner side, and half under the gland on the lower, outer side.

 

WHAT HAPPENS NEXT?

After breast augmentation, there is a slight swelling which disappears after a few days. Prescription pain medication is prescribed at the second consultation. You will need to wear a special bra, which will be prescribed at the second consultation. This bra helps to combat the swelling and keep the implants in the right position.

Generally, most patients can resume their daily activities, including work, after one or two weeks. You will need to refrain from strenuous exercise or lifting heavy objects for about six weeks.

RESULTS

Dr. Sylvain David will show you your breasts as soon as you leave the clinic, either the same evening if the operation is performed on an outpatient basis, or the next morning if you stay overnight. In this specialized center, there are only single rooms. After breast augmentation, your breasts will be fuller and better shaped, and asymmetry will be treated in the best possible way. Once the initial swelling caused by the surgery has subsided, you'll see that the results are more natural.

The incisions made during surgery generally fade over the following month to become fine scars. It should be noted that the total healing process lasts 1 year. After 15 days, you no longer need to apply a dressing, but the aesthetic improvement of the scar lasts 1 year.

The convalescence and healing period is when you'll have your biggest role to play. I'd like to stress the importance of following all post-operative recommendations and instructions.

Possible complications:

Fortunately, complications are rare in this type of operation.

The most frequent risk, but still less than 1%, is haematoma. This is bleeding inside the prosthetic cavity that appears after the operation "within 24 hours", in which case it is necessary to return to the operating theatre to evacuate the haematoma.

Prosthesis rupture or the appearance of a shell is rare, but increases with age.
